This is SKR 32, anti-submarine frigate, by NATO known as Mirka class, by Soviets as Project 35. Part of the 26th Anti-Submarine Warfare Brigade based at Baltiysk. Built in 1964 at Kaliningrad and stricken from the Navy list in 1989.

Looks like an old PETYA-class corvette.
There were several ships of this class including their modifications: PETYA I, PETYA I mod., PETYA II, PETYA II mod.
