1949 PROVIDENCIA Providencia Shipping Co., S.A., Panama [Fratelli Rizzuto, Genoa]
1963 PROVIDANCE Providencia Shipping Co., S.A., Beirut [Fratelli Rizzuto, Genoa]
1965 Stranded 21. Oct 1965, 4 miles NW of Capo Santa Croce, Sicily, voy. Augusta - Genoa, in ballast, subsequently refloated, sold for demolition.
1967 Arrived at Split 1967 August for demolition.

Surprisingly it appears to be a genuine 7-digit LR number (probably because it was still lingering in the LR system at the time, despite having just been scrapped. The first edition with the 7-digit numbers was LR 1969-70.

No, she was not built as a tanker. Matson Line had several cargo ships of this configuration.

That can't be right - IMO number were assigned only starting from 1970.
