Long time ago had the same in Colombo. Picking up two 20ft with 40ft spreader and when they came out of the cellguide....there they went down again and lots of damage.

Not sure but could be the two units were discharged with spreader in 40 position with center corner castings not locked. Spreader are supposed to be fitted with TTDS (two twenties detection system) in order to prevent this from happening.
