Gantry collapse at Southampton container terminal. Operator decided to clean his windows as the gantry was booming down, the cables tangled and the boom collapsed.

Please get your facts right before commenting, the official reason for the failure of the boom hasn't been published.
As for the Crane Operator's position when the boom came down, it had no bearing on the failure of the crane, and in fact saved his life, as if he had been at the boom station, this ended up being taken down with the boom.

 Re: Kyoto Express
If the operator would of been at his station he would of noticed the the problem and potentially avoided the boom collapse! Fair enough it had saved his life as a result of the operator not being in his station however from a HSE & operating point of view he should of been at his station when the Gantry was booming down to avoid situations like this from occuring. It was also lucky that none of the Ships crew members were not injured in this accident.

 Re: Kyoto Express
It was quite likely a structural failure, and he wouldn't have any way of stopping the boom collapse.
There are a lot of us on here that work in the Industry.
As for Crew Members not being hurt, thank God NO-ONE was injured or worse, killed.
It was a quite unprecidented event that we hope will never happen here again.
