She's the Patrol Frigate GC-11 "Almirante Clemente", ex-D-12, an old Almirante Clemente Class Destroyer Escort built by Luigi Ansaldo Ficantieri in Italy. She was commisioned in 04-12-1956 and she's officially still in active with the Venezuelan Coast Guard.

I saw her in nov 08, close up, she has massive holes in the superstructure
and all what was usable on deck removed. Now it is actually a empty shell with no crew

ALMIRANTE CLEMENTE GC-11 (ex F-11, ex D-12)
builders: Ansaldo, Italy
First of a class of six vessels completed during 1956-57.
This one and GENERAL JOSE TRINIDAD MORAN (GC-12, ex F-12, ex D-22) refitted during 1969-76 (delayed by problems) by Cammell Laird/Plessey. Re-engined by CNR, Italy with diesel engines in 1984-85. Both operated by Coast Guard since 1986.
